relinquish him/her to ___________________________________________________ volunteer for _____________________________, who will, at her discretion, decide what is the best course of action to take with this dog. I am entering into this agreement with full knowledge that the relinquishment of this dog is irrevocable. I am giving up all title and possession of the dog and understand that I will have no actions of any kind against _____________________, the _________________________ or any of its representatives for any reason arising out of this agreement.

I further warrant and represent that this dog has never bitten me or any other individual and I understand that __________________________ is relying upon this representation.
